I just finished Primal for who knows how many times. This time it was 100% completion on hardest settings (Expert Survivor Permadeath), and the trick was that I had to carry the Olympic fire with me = have a club on fire all the time and use only the club(s) as your weapon. It was a lot of fun, and a lot of sneaking around and burning stuff.
I originally started on expert and it got borderline frustrating due to the lack of defensive options in melee. I think hard would be a good balance. Yeah the melee combat leaves a lot to be desired. I've played almost exclusively on Expert (and Survivor and Permadeath) and I'm satisfied with it. TLDR: the combat is inherently riskier than other far cry games, and especially in the early game you can get into nigh-unwinnable fights by aggroing certain animals. Unless you want to spend the whole early game (i.e. until you can ride a sabertooth) crouch walking across the map, I would advise against permadeath. #1.
